Complaint/Grievance Procedure
Olympic Sports & Spine (OSS) prohibits retaliation against any patient, companion, employee, or visitor because they opposed or complained about discrimination in good faith, filed a grievance, or participated in a discrimination charge or other proceeding under federal, state, or local anti-discrimination law.
- Complaints may be submitted at any time, though the earlier you submit a complaint, the earlier OSS can address your concerns.
- You may file a complaint through one or multiple of the following means:
Email: compliance@osstherapy.com
Phone: 253-581-5200
Webform: www.osstherapy.com/contact
Mail: OSS Compliance Officer, 6050 Tacoma Mall Blvd, Suite 300, Tacoma, WA 98409
- The OSS compliance team will conduct a thorough investigation or each complaint received. The compliance office will issue a written response within thirty (30) days after receipt of the complaint. If additional investigation time is required to complete a thorough investigation, notification will be made of this need in a written response no later than 30 days after receipt of the complaint.
- The person filing the complaint may appeal the decision. Appeals should be made in writing and a written decision in response to the appear will be provided within thirty (30) days.
- The availability and use of this procedure does not prevent a person from filing a complaint of discrimination with the U.S. Department of Health and Human Services or Office of Civil Rights.
